DACC is proud to launch our new Home Visitor Pilot Scheme in April 2023.
Working in partnership with NHS Primary Care we will prioritise patients referred by local GP surgeries who have high levels of need, lonely, housebound, bereaved and/or isolated.
This project will be in addition to our centre based day care and will provide people aged over 70 home visits to offer support and to make sure they are
safe, well and fed.
We provide day care services for people living in County Durham. Our services are Dementia Friendly and based in Bishop Auckland, Gilesgate, Pelton, New Brancepeth, and Willington.
