Hello,
DACC has launched a Home Visitor Scheme. This reaches out into the community and offers a home visit to anyone aged 60 and over who is housebound, lonely and isolated. Please ask your GP practice to refer you. This is free of charge.

Home Visitor Scheme Pilot 2023
Our findings show:

- New title
- In the first 12 months, 46 people were referred by GP Social Prescribing teams as lonely and needing support .
- 547 home visits were made in the first 12 months.
- 210 calls made.
- Average age is 83 year old.
Key Themes included;
- Loneliness
- Poor Mental health, suicidal
- Low Mood,
- Communication issues
- Bereavement/Grief
- Struggling with practical tasks such as telephone and making appointments.
- Food shopping
